Register as a patient - living outside the practice area

From January 2015 Kingskerswell and Ipplepen Medical Practice are able to register new patients who live outside of the practice area without any obligation to provide home visits when the patient is unable to attend the Health Centre. The registration of a patient who lives outside the practice area however has to be cleared by a GP as being clinically appropriate. For the GP to assess whether your registration is appropriate they need the following information.

This practice does not accept prescriptions over the telephone. Therefore if you live outside the area you must be prepared to either order prescriptions over the internet on SystmOnline, present at the practice with a repeat slip or post your request.

Have you ever been housebound?
Have you ever been under the care of the Community / District Nurses?
Do you have any long term / chronic health conditions?
Are you any repeat medications?
Confirmation needed
